Medicare Savings Program & Medicaid Application Assistance for the 65+, Blind and Disabled Populations

Health & Welfare Council of Long Island’s Facilitated Enrollers will be at the library to provide enrollment assistance. The Facilitated Enroller will help clients fill out all applications and collect required documents to submit to the Department of Social Services. They can assist with the following insurance applications:

  • Access NY Healthcare Medicaid Application
  • Supplemental A Application
  • Medicare Savings Program Application
